MOUNT AIRY — Mr. Jack Ray Tate, 59, of 3236 E. Pine St., Mount Airy, husband of Mrs. Barbara Jean Hayden Tate, passed away at Forsyth Medical Center. He was born Nov. 17, 1952, in Surry County to the late Ray and Hazel Davis Tate. He was employed by Walmart. Surviving are his wife, Barbara Jean Tate of the home; two daughters, Peggy Velazquez and Mark of Irving, Texas, and Patty Tate and Stephen Moose of Mount Airy; a son, Barry Tate and Wendy of Lancaster, Pa.; a grandson, Corbin Moose of Mount Airy; mother-in-law, Louise Hayden of Mount Airy; sisters-in-law, Dianne and John Willey of Lowgap; and brothers-in-law, Gary Hayden and Joanie of Mount Airy and Earl Hayden and Crystal of Westfield. A funeral service will be held at 2 p.m. Wednesday, March 21, 2012, at Moody Funeral Home Chapel conducted by the Rev. Mark Reece. The family will receive friends from 1 to 2 p.m. Wednesday at Moody Funeral Home and other times at the home Patty Tate, 1117 Linville Road, Mount Airy.
